Impalement Hazard

Hazard:

The vertical rebar protruding from this concrete formwork is an impalement hazard. This hazard can result in very serious injury. VSCF&S workers had been exposed to the rebar and numerous trip hazards during installation of the Flexible expansion loop. VSCF&S technicians notified the GC that there were no implement caps on the rebar, but no corrective action had been taken.

Corrective Action:

Immediately stop work and notify the general contractor of any serious hazard. Supervisors must document the name of the GC representative they spoke to with the date & time and contact the VSCF&S District Manager. If a camera is accessible take a picture of the hazard.

Other examples of potentially serious injury are: falls, electrocution, struck-by, confined space, and trench. All VSCF&S employees are encouraged to report unsafe conditions or acts you are exposed to on any job site.
